What the form should carry

• Animal name, species, breed and current weight.
• Owner name.
• The preparation requested — active ingredient, concentration, dosage form and flavour.
• Quantity, directions and refills authorised.
• Veterinarian name, licence number, and DEA number where applicable.
• Signature and date.

Weight changes

If the animal’s weight has changed since the original prescription, please give us the current figure on the authorisation. Most veterinary preparations are dosed by weight and we would rather re-check the dose than repeat the last one.
